We all know death in the game isn't done what so ever, so here is what I've come up so far. I'll be updated this.

When you die, there should be multiple ways to get back. After death, you turn into a soul and are transported to a certain area (to be decided) where you float around as a soul. Your body (corpse) is kept where you died. There are a few ways to get back in your body.

-Sacrifice loss of mind (exp/etc) to get a new body, where you must go back and get the items from your corpse.
-Have someone in the mortal realm "embed" a shard into your corpse, which will cause you (the soul) to be yanked back into the mortal realm in corpse.
-Travel to the mortal realm as a soul and integrate back into your corpse.

Comments/ideas/etc are welcome.

I'd add a two more ways myself based off the anime but I don't think I should post what they exactly are due to spoilers.

Since death happens quite regularily, esp. to beginners and lowbies, having special circumstances like having some one else embed a shard in your corpse to revive you and then have to trust you to give it back each time you die is ridiculous. I like the spirit idea, where you float around, maybe for a while before a soul catcher picks you up and takes you to your corpse. Or maybe leave it as it is, cause it is still damn annoying trying to refind your corpse and lose a few mil exp when you die.

Also, concerning death, humans should be able to learn a skill called something like "corpse seal" which can preserve a players corpse from decomposition and looting from anyone but that player...

It's not required, like I said there'd be multiple ways to get back. Anyone who doesn't have a friend/etc can just sacrifice mind (exp etc) to get back.
